Take Shapes was developed by an occupational therapist who specialised in treating dementia and stroke patients. She found that existing assessment tools made patients feel anxious and failed to provide the full range of
information she needed to create effective, individually-focused care plans. So she created Take Shapes.
In ‘Living well with dementia: A National Dementia Strategy’, the government highlighted the need for change in dementia care. Its recommendations include putting people and their carers at the heart of planning their lives and delivering services based around the needs of individuals and families.
